Where can I get the forms?

The Bronx County Clerks Office does not furnish the necessary forms for any of the Business related services. These forms may be obtained at stationary stores that sell legal forms. We recommend that you purchase three forms when filing a Business Certificate, one form will be filed in our office as the original, you will need one form to present to the bank if you plan on opening a business account and you should retain one form for your records.

*Please Note: Our office will not accept any certificates if corrections have been made or if white out has been used. All forms must be typed or printed in blue or black ink

After I purchase the form what do I do?

Before choosing your Business Name and filling out the forms please perform a Name Search at our office to assure that your chosen name is not already in use. Once the forms have been completely and accurately filled out they should be notarized. If the business is a partnership all of the partners signatures have to be notarized. Our office offers a notary service for your convenience providing you have proper identification. Once notarized, a name search must be conducted in our office (Miscellaneous Records Dept.) and the form is to be filed with the Notary Department.

What are the fees?

  • The fee for Filing an Amended Business Certificate is $100 - Plus $10 for each additional certified copy you may require. (Payment must be in cash, money order or certified check)
